The Project consists of a home-based care service in the city of Novara together with No-Profit Organization NOVARA SOCCORSO for elderly or otherwise vulnerable individuals. It provides recurring visits by qualified personnel in order to monitor their vital parameters as well as their overall health condition. The Project identifies and manage socio-health situations to compensate for the current shortcomings of the welfare system creating a free support network that can facilitate access to local resources. Main objective: protect frail elderly individuals without causing trauma by removing them from their home and familiar environment, to PREVENT deterioration of the condition(s) they are affected by, connecting them at an early stage to the local care network. The Service focuses on PREVENTION, social and motivational support for the elderly and their families through the training of Volunteer ensuring the continuity and SUSTAINABILITY.
